Completing your Pre-Appointment Forms

Summary: Before your Skin Clique appointment, you may need to complete intake or consent forms. Here's how to access and complete them.

How to complete your forms

1. Go to skinclique.com and sign in at the bottom of the homepage.
2. Open your profile and go to Appointments > Upcoming Appointments.
3. Select your upcoming appointment and complete any forms attached to it.

Forms must be completed before your appointment time. If you're unable to complete them in advance, your provider can assist you at the start of your visit.

Didn't receive a confirmation or form link?

If you didn't receive an appointment confirmation email with a link to your forms:

1. Log in directly at skinclique.com and check your upcoming appointments.
2. If your appointment isn't showing or forms aren't available, submit a support request with your full name, email address and appointment date and location.

Tips

• Complete your forms as early as possible — they help your provider prepare for your visit.
• If you have questions about what a form is asking, contact your provider directly.
